DEPARTMENT OF JUSTICE I OFFICE OF THE INSPECTOR GENERAL MEMORANDUM OF INVESTIGATION Case Number: 2019-010614 Reporting Office: New York Field Office Surveillance Video Analysis of the Metropolitan Correctional Center New York, New York, Special Housing Unit On March 7, 2022, Special Agent (SA) =, U.S. Department of Justice (DOJ) Office of the Inspector General (OIG), reviewed the Federal Bureau of Prisons (BOP), Metropolitan Correctional Center (MCC) New York, New York, surveillance camera footage of the MCC New York Special Housing Unit (SHU) from August 9, 2019, at 4 p.m. through August 10th, 2019, at 7 a.m. SA =, Federal Bureau of Investigation (FBI), New York Field Office (NYFO), conducted the attached review of the same video footage and provided it to the OIG. The OIG's review was conducted as part of the OIG's investigation into the events surrounding the death of inmate Jeffrey Epstein, Register Number 76318-054, who was found unresponsive in his assigned cell Z06-220 on the L tier within the SHU. The camera footage was extracted by the FBI lab located in Quantico, Virginia, from the MCC New York surveillance camera video arrays seized by the FBI on August 10 and 11, 2019. The documents utilized by the OIG to assist with its analysis has been attached to this report. The MCC New York video surveillance footage has been saved separately to the case file due to its large file size. The review and analysis produced the following results. Background The MCC New York 9th floor is split into two housing units, known as 9 North and 9 South. The MCC New York's Special Housing Unit (SHU) is located in 9 South, and consists of 6 tiers, including G, H, J, K, L and M tiers. Unit 10 South (10S) is located above the G tier in the SHU, and is designated for Special Administrative Measures (SAMS) inmates. Unit 10S can only be accessed via Door 46, which is located in the SHU. During interviews conducted by the OIG, multiple SOP employees assigned
