A Sikorsky Company July 22, 2011 Guardian Jet, LLC 1445 Boston Post Road Guilford, CT 06437 Attention: Ted Osgood, Aircraft Sales Director Dear Ted, AAG has inspected Aetna's S-76C+, serial number 760472 (N 62AE) to the extent requested and with the exception of the items listed below, we find the helicopter to be in airworthy condition as delivered. Items discussed with you that required closer examination and have been cleared are: • Main Wheel Brakes: Within limits for continued service. • Rotor Brake Disc: Within limits for continued service (some wear grooves present but average wear is .499" thickness; limit is .495"). The items of concern are: • No. 2 Engine 15 Indicator 'Press-to-Test' function inoperative. Estimate $1200 to $3000 for repair depending on internal condition. Several days turnaround at best. • One Main Wheel has one missing tang for the brake. If we remove to inspect further, the wheel needs to be overhauled, or replaced if beyond repair. Expect up to $6000 for new wheel and tire. Could be less if repairable, but new tire should be installed anyway. • Both engine exhaust ejectors and prolongators (4 pieces total) are worn through and need welded repair. Estimate $500 each, plus shipping. Turnaround is one to two days. Note that if disassembled for repair, the nutplates and clips will be destroyed due to their condition and replacement parts and labor is estimated to be $800 per engine. • Nose Landing Gear Actuator reported as leaking statically. This is not uncommon, nor beyond limits. It should be monitored and cycled and if persistent or if leakage increases, repair or replace the actuator. Estimate $6000 for parts plus several labor hours. Lastly, the SOAP samples were taken prior to the start of the inspection and Fedexed immediately, and generally take a week to process.
